This form is only valid if you have completed your TeachMT application elements including transcript submission through OPI's website.  Applicants who have not completed their applications elements will be removed from the queue.

 

We look forward to this recommendation, but they do take time, and the queue sometimes fills up rapidly.  For this reason, please do not submit more than one request.  A second request does not speed up the process and can confuse our office.  To initiate a university recommendation, please complete the entire educator licensure request form below:
