Thank you for your participation in this survey. Your contributions will help us better understand why over 30% of working aged cancer survivors are exiting the workforce within 5 years of diagnosis. This research will be used to communicate ways employers can better support post-treatment cancer patients.
You will be led through a series of questions that will collect demographic data and information about your work status at the time of diagnosis and after treatment. You may skip any question you do not wish to answer. Your information is completely confidential. By submitting this survey, you are consenting to participate in this study.
