NACOE Academic Suspension
Appeal Document Preparation Survey
The questions in this survey align with those on the University Scholastic Appeals Board (USAB) cover sheet. Responding to these prompts will help you prepare a suspension appeal document for submission to the USAB. Contact the NACOE Dean's Office (406.994.2272) to obtain a copy for review and edit prior to meeting with the NACOE Assistant Dean to review your suspension appeal materials.
Through these suspension appeal materials, the USAB would like to understand:
1. The circumstances that contributed to your academic underperformance, including:
- Identified and described actors that caused or contributed to your academic challenges.
- Clearly explain how each factor prevented you from succeeding academically.
2. Your plan for academic success.- Describe the steps you have taken to address the reasons for academic underperformance.
- This survey will help develop an Academic Recovery Plan which will be your strategy to ensure academic success upon your return.
- Focus mainly on how you will succeed in the courses in the term ahead; although a vision for where you wish to go with your academic achievement can be helpful in identifying success strategies for the following semesters.
3. Statement of Academic Purpose and Plan.- Communicate goals or a vision for your college education.
- Include evidence of academic potential.
- Provide a planned course schedule for the next two academic semesters. Include these in your DegreeWorks plans titled "Recovery Year Plan," and in this survey.
- Establish and describe three SMART Goals. These are Smart, Measurable, Assessable, Realistic, and Timely goals to help you focus effort and engagement.
While not required, you may wish to submit additional documentation in support of your request. These items should be additional information you believe will assist the USAB in making its determination. These items may include:
- Letter of reference from an MSU faculty member or advisor you have worked with to address academic challenges. The letter should address academic potential, the ability to follow through with the detailed Academic Recovery Plan, and suggested areas for additional academic support.
- A statement addressing any current or pending MSU Code of Student Conduct violations, including how violations impacted the student's academic success, and how you are working to address issues.
- Documentation from the Office of Disability Services, if applicable, that relates to the ability to be academically successful.
- Any additional documentation believed to be relevant to this appeal, including information related to extenuating circumstances believed to have contributed to academic underperformance.
When this survey is completed contact the NACOE Dean's Office to obtain a copy for review and editing. Add the completed Suspension Appeal Cover Sheet and additional documentation you have chosen to include. Schedule an appointment with NACOE Assistant Dean, Dr. Jennifer Clark. You will be expected to discuss the circumstances leading to your suspension, the circumstances prompting your appeal, and your academic plan to ensure your future success.
You must provide all suspension appeal documentation at the meeting with the NACOE Assistant Dean so they may be uploaded for the USAB system for review. Please note: "GID" means your MSU Student ID.
